Our focused conditions are Diabetes,Cardiovascular Disease including HTN (hypertension) and COPD. We also manage Transitions of Care with those patients that have experienced an exacerbation of or a new onset diagnosis.
Our Disease Management services are supported or enhanced with FDA approved biometric monitoring devices whereby the patients values are transmitted in real time.
The goal of this program is to improve health literacy, improve self managed care and to support the efficiency and effectiveness of the treating physicians plan of care.
