Completion of the Medicare Planning Fringe Benefit® Inquiry Form provides the information necessary to estimate future employee benefit cost savings associated with implmentation of the Medicare Planning Fringe Benefit®. 1) The company's most recent IRS 5500 report data is used to collect census data of active & retired health plan members. 2) Reasonable assumptions are made regarding the number of Elgible Plan Members. Accurate EHP cost sharing data for premiums, deductibles and MOOP are required to compare the known cost of the EHP with actual Medicare plans associated with the EHP address provided in the Inquiry Form. This information is compiled into a Medicare Planning Fringe Benefit® Pilot Project Proposal for consideration by the EHP key decision-makers.
