Abacus Health Solutions offers three chronic condition management programs to support health plan members with diabetes, hypertension and/or obesity/GLP-1 use. Our data driven programs result in improved health outcomes for participants and savings for the plan sponsor. The programs include a timely, behavior-contingent, zero-dollar pharmacy copay incentive for covered medications and supplies for the condition being treated. The programs are elegant in their simplicity. Participants must meet and maintain program requirements to earn the copay waiver. If a participant is not adherent, the waiver is removed, and the copay is reinstated. Abacus does not prescribe medications. The program framework encourages routine visits with a primary care provider to complete evidence-based standards of care and obtain recommended prescriptions. A typical Abacus client is a self-funded group with 500+ employees/1,000+ health plan members. Abacus is a SHRM Recertification Provider.
